Elderly Man Found Dead in Assisted Living Facility Shock

A woman made a distressing discovery at an assisted living facility where she had recently placed her elderly uncle. Angela Boorn found her 75-year-old uncle, Gerald McClellan, deceased in a chair, with authorities confirming he had been there for at least three to four days.

Boorn expressed shock and devastation at finding her uncle in such a state, emphasizing that he had not been checked on for multiple days. She described the scene as resembling a skeleton, with hollowed-out eyes, recounting her usual routine of entering his room and announcing her presence, but receiving no response on the day of the discovery.

McClellan was residing at the LifeStream facility in Sun City, approximately 16 miles from Phoenix. Boorn had moved him there in January after his wife’s passing, believing the facility had adequate safeguards in place. The facility had a daily check-in system where residents would signal their well-being by pressing a button in their rooms each morning.

Boorn highlighted the failure of staff to follow the protocol despite McClellan not triggering the check-in button for several days. She urged others with family members at the facility to be vigilant and ensure their loved ones were being properly cared for.

While it is unclear if Boorn plans to pursue legal action against LifeStream, the facility issued a statement expressing sadness over the resident’s passing and extending condolences to the family. The incident has raised concerns about the level of care provided at the facility, prompting a call for increased vigilance from family members of residents.
